Home Safety & Functional Assessment​
Our healthcare and rehabilitation professionals can evaluate mobility, activities of daily living, falls risk, equipment requirements, and environmental safety.
Equipment & Assistive Device Navigation​
We can assist clients with identifying appropriate mobility and assistive equipment and provide support navigating programs such as Ontario's Assistive Devices Program (ADP), where applicable.
Care Planning
Assessment findings can be translated into an individualized care plan identifying recommended services, frequency, goals, responsibilities, and monitoring requirements.
